HCDI’s approach to addressing health disparities in a deliberate, data-driven manner is paramount to achieving health equity. We consistently employ research-based methodologies like our Parachute Model to provide online, non-downloadable, Internet-based software applications for medical data collection and patient education to build strategic alliances, apply innovative communication strategies, and conduct social marketing to influence positive consumer behaviors. As a healthcare methodology, the Parachute encompasses four key pillars, Research, Communication, Strategic Alliances, and Community Relations utilizing our Caring For Your Health (CFYH) e-Tool. We conduct an environmental scan of the targeted geographic location characterized by adverse health outcomes and associated high costs. All of our communications with stakeholders and the target population are respectful, culturally sensitive, and linguistically appropriate and, if necessary, written materials are 508 compliant. We value our strategic alliances and engage stakeholders from within the communities we serve. We engage patients, families, and primary caregivers in an effort to disseminate information to increase awareness, educate, and change behavior.

By using a Human-Centered Design approach to problem-solving, HCDI is able to reach vulnerable populations, become a trusted partner, and provide person-centric solutions. By taking the human perspective into account during all steps of the problem-solving process, we are able to create more culturally and linguistically appropriate human-informed solutions. These solutions have been designed with the end-users, therefore driving higher engagement and growth with the target audience.
This approach starts with people and ends with innovative, evidence-based solutions. By creating an open line of communication between the user and us, we are able to better understand their needs and create a solution tailored to them. By involving the human perspective in all steps of the problem-solving process, HCDI is able to customize our client needs in a cost-effective and sustainable way.
