HealthCare Dynamics International (HCDI) is a Care Transformation Organization (CTO) that participates in the Maryland Primary Care Program (MDPCP). The MDPCP is a voluntary program that provides qualified primary care providers with funding and support to transform the quality of care provided to patients while reducing spending. The transformation allows primary care providers to play an increased role in the prevention and management of chronic disease and unnecessary hospital utilization.
