All de-identified or aggregated information shall be owned exclusively by HCDI.

We share your protected health information with physicians and other covered entities as permitted under the Health Insurance Portability and Accountability Act (HIPAA). All protected health information shall be obtained and used according to the terms and conditions of any applicable Business Associates Agreement, as well as in compliance with HIPAA and other applicable laws and regulations.

In some instances we may retain other companies and individuals to perform functions on our behalf, including, but not limited to, web hosting platforms.
